Abstract

With the increasing number of patients with diabetes, and with the increasing complexity of care, managing the care of patients in a co-ordinated effective and efficient way is an increasing challenge. Data linkage of health-care databases using a single patient identifier has enabled us to establish a comprehensive patient management system. Originally developed as a research programme (DARTS) looking at epidemiology and patient outcomes, it is now a patient management system acting as a common repository of clinical information for all health-care professionals involved ion the care of patients with diabetes.
