The results of 170 consecutive bimedial rectus recession operations performed for alternating esotropia are reported. Successful outcome was achieved in 54% of cases, but a higher success rate of 64% was obtained where there was an associated accommodative element or a high accommodative convergence: accommodation ratio, and 70% success was obtained when the angle of deviation was 20° or less. The results justify bimedial rectus recession for selected cases of alternating convergent strabismus.
