Abstract
Modern hospitals are complex institutions that treat increasingly unwell patients with multiple co-morbidities. The aim of such institutions is obviously to improve the outcome of the patients they treat. Unfortunately, this goal is not always achieved and some patients are inadvertently harmed. Studies in the U.S. conducted more than 30 years ago reported that patients admitted to hospital suffer serious adverse events unrelated to their admission diagnosis or underlying medical condition.
